- Submit these documents at a DoT licensing centre:
- Get a medical assessment – book an appointment at your GP and complete your Medical Assessment Instructions Form and Medical Fitness To Drive Form. If you don’t have a preferred medical professional, go to the local Uber Greenlight Hub and ask for the current list of partners.
- Pre-medical assessment form – you can print and complete before your medical assessment, or during your appointment with your GP. These documents will then be sent to the DoT.
